In compliance with the provisions of the Clean Water Act, as amended, (33 U.S.C. 1251 et. seq; the "Act"),

is authorized to discharge from a facility located at 316 Maxwell Ave, Maxwell, in Colfax County, New Mexico, to unnamed dry arroyo, thence to Canadian River in Segment No. 20.6.4.305 of the Canadian River Basin.
The discharge is located at the following coordinates: Outfall 001: Latitude 36° 31' 55" North, Longitude 104° 32' 16" West,
in accordance with this cover page and the effluent limitations, monitoring requirements, and other conditions set forth in Part I, Part II, Part III, and Part IV hereof.
