This form is required to gain access to the CAMD Business System (CBS). Users must submit a hard copy of the form with a wet signature via overnight or postal mail.

Many EPA staff are working remotely due to COVID-19.  Users must email their Electronic Signature Agreement form to CAMDforms@epa.gov to ensure prompt receipt and processing.  The hardcopy form must also be mailed to EPA for our records
