Instructions for How to Complete Self-Identification and Other Certifications in CDX for EPA-Initiated Risk Evaluations

The documents below illustrate the CDX reporting  and amendment process for businesses subject to fees for EPA-initiated risk evaluations. The first set of instructions shows how to self-identify as an entity subject to fees, certify that your business does not manufacture the relevant chemical, or certify that your business has ceased manufacture of the chemical before it was prioritized for risk evaluation. The second set of instructions details the process for amending a previously-submitted self-identification or other certification.  More information on the process for identifying manufactures subject to risk evaluation.
